Whittier Rehabilitation Hospital - Transitional Care Unit - Haverhill MA
Clinical Nurse Manager


Regular Schedule will be M-F days (40 hours), with coverage as needed on weekends and off-shifts.

Pay: $42 - $48


We are seeking a Nurse Manager to oversee and lead our Transitional Care Unit housed within our beautiful Rehabilitation Hospital on Ward Hill in Haverhill MA.


Essential Duties:
  1. To provide exceptional care to all residents including but not limited to:
  2. Under the direction of the Director of Nursing Services, holds 24-hour authority, responsibility and accountability for the clinical management of assigned resident/patient care unit.
  3. Obtain report from nurse being relieved and records sufficient information to implement appropriate follow up action as necessary.
  4. Provides report to nurse coming on duty, including sufficient information for follow-up action if necessary.
  5. Conducts resident rounds, reporting problems to nursing supervisor and initiating corrective action.
  6. Administers and documents medication, enteral nutrition and treatment per physician order and accurately records all care provided and observations daily.
  7. Performs comprehensive assessments of each resident on unit.
  8. Develops care plan based on each resident's assessment.
  9. Obtains physician orders and any revisions for all treatment and medications needed.
  10. Assesses on a weekly basis the skin condition, stage, size, site, depth, color, drainage and odor of condition.
  11. May conduct staff meetings; coordinate services rendered by nursing personnel with those rendered by personnel of other departments.
  12. Observes infection control policies and procedures relating to nursing services.
  13. Directs and supervises activities of professional and non-professional nursing personnel.
  14. Supports and participates in In-services Education Programs.
  15. Renders immediate assistance to any resident in need.
  16. Provides clinical supervision of nursing assistants providing direct patient care.
  17. Assumes responsibility for compliance with federal, state and local regulations within the assigned unit.
  18. Counsels nursing assistants and communicates clearly with Director of Nursing if issues arise.
  19. Collaborates with interdisciplinary team to assure that high quality resident care is maintained.
  20. Prepares nurse assistant assignments and explains all pertinent resident care.
  21. Supervises distribution of nourishments.
  22. Supervises the removal of all postural supports.
  23. Orders, receives and documents medication according to facility procedure.
  24. Supervises response to residents calls for assistance and responds to call lights as necessary.
  25. Insures that all physician renewal orders and physician documentation is timely and appropriate.

Prerequisites:
  1. Graduate of an accredited Registered Nurse school of nursing.
  2. Current and active Nurse license in the state of Massachusetts. RN strongly preferred.
  3. Current certification in Basic Cardiac Life Support in accordance with AHS or ARC standards.
  4. Minimum of three (3) years recent clinical experience preferably in Gerontological or Rehabilitative Nursing.
  5. Prior MDS Management experience preferred.
