Job Description Summary
Manages individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment, and services through the successful coordination and completion of processes including assessment, planning care, and providing care, in accordance with departmental and hospital policies and procedures.
Job Description
- Assessment/Reassessment
Assesses and diagnoses patient and family needs to provide quality care by:
a - Initiating admission assessment within 8 hours of admission, identifying and documenting patient/family/significant other teaching needs throughout hospitalization. - Plan of Care
Develops, discusses, and communicates a prioritized problem list/plan of care for each patient, in collaboration with support persons to address all needs.
a - Develops, evaluates, and updates individualized plans of care, documenting outcomes.
b - Communicates plan of care with the primary nurse.
c - Facilitates discharges/transfers by 12:00 pm, communicating barriers via Chain of Command.
d - Documents all patient care following nursing policies, standards, and indicators.
e - Assesses/reassesses and documents patient's comfort levels using appropriate scales and responses to interventions.
f - Advocates for patient/family, seeking help when needed.
g - Raises ethical concerns with the clinical team and utilizes resources for ethical decision-making. - Medication Administration
a - Follows policies for administering, transcribing, and recording medications.
b - Properly documents narcotic withdrawal, administration, verifies counts, and resolves discrepancies.
c - Initiates and maintains IV access per policy. - Safety
Fosters a culture of safety through personal ownership.
a - Verifies patient identity with two identifiers before invasive procedures.
b - Complies with CDC hand hygiene guidelines and universal precautions.
c - Assesses fall risk and implements interventions.
d - Ensures the environment meets regulatory standards.
e - Understands roles during hospital codes/emergencies.
f - Practices patient handoff using SBAR principles. - Professional Development
Owns professional growth by incorporating evidence-based practice and research.
a - Obtains at least 5 contact hours annually.
b - Practices within MA Nurse Practice Act.
c - Participates in unit/hospital initiatives.
d - Understands roles as charge nurse, preceptor, mentor.
e - Analyzes problems, evaluates solutions, communicates effectively.
f - Keeps informed about hospital policies and equipment.
g - Interacts compassionately with patients and families. - Technology
Embraces technology in work practices.
a - Uses Lotus Notes for email and resources.
b - Utilizes Meditech applications as required.
Essential functions include ongoing learning and technological proficiency.
Job Requirements
Minimum Education: Graduate of an accredited School of Nursing
Minimum Work Experience: Previous acute care RN experience preferred
Required Licenses/Registrations: RN - Registered Nurse
Required Classes/Skills: ACLS - Advanced Cardiac Life Support
Additional Knowledge: Proficiency in acute care nursing, knowledge, and skills.
