Description

We're looking for a compassionate registered nurse to join our team!We're a Home Health Agency headquartered in Hyannis, MA, providing nursing and patient care services to critical regions throughout Massachusetts. We take a sensitive approach to healthcare, meeting patients where we can support them most - at home.

RESPONSIBILITIES/ESSENTIAL FUNCTIONS

  • Demonstrates proficiency with nursing skills in preventative and rehab nursing procedures.
  • Coordinates total plan of care with other healthcare professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
  • Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within state specific guidelines, and submits accurate and complete documentation in collaboration with physician orders in the required 24 hour time limit.
  • Performs patient comprehensive socio-psychological assessments at designated time points and develops the appropriate plan of care/discharge.
  • Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.
  • Performs home visits to clients in designated territory
  • Completes head to toe assessments
  • Performs injections, wound care as needed
  • Distributes medication management
  • Participates in: the Performance Improvement Committee process as requested; required monthly staff meetings; and assisting in the planning, operation and evaluation of the nursing services of the organization.
  • Determines medical necessity for other services
  • Submits requests for re-authorization of “more visits needed” to payers within time guidelines.

SPECIAL EQUIPMENT TO BE OPERATED

  • Vehicle to do routine patient visits. Possible transfer/lifting devices, medical supplies

WORK ENVIRONMENT

  • Indoors, performing patient care assessment/tasks in the patient’s residence
  • Travel to patient residences/meetings; will require transportation to get to various work sites
  • Possible exposure to communicable diseases

FUNCTIONAL ABILITIES

  • Able to access patient’s homes that may not be handicap accessible.
  • Must hear, speak and write in English fluently.

PHYSICAL REQUIREMENTS

  • On a daily basis: walking, standing, sitting, reaching, use of telephone, use of computers & other office equipment
  • On a daily basis: able to lift/transfer 150 lbs or greater, bending, stooping, climbing stairs.

  • Graduate of an approved professional nursing program
  • Currently licensed “in good standing” in MA
  • Minimum of one year experience as a professional nurse,preferably in home care
  • Must have current CPR
  • Must have criminal background check
  • Must understand and communicate fluently in English
