  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and interdisciplinary utilization.
  • Ensure annual outcomes meet National Benchmarks.
  • Refer to community resources as appropriate.
  • Evaluate the clinical needs of the patient; develop, implement, and revise the patient's plan of care; address all problems and document accordingly.
  • Participate in interdisciplinary patient case conferences; evaluate the effectiveness of the care plan.
  • Instruct and supervise Home Health Aide plans of care every 14 days with documentation.
  • Notify MD/other care providers of significant patient condition changes.
  • Complete admission and other paperwork timely.
  • Provide clear, concise narrative documentation reflecting skilled care, teaching, direct care, and patient/caregiver responses.
  • Obtain and clarify valid MD orders for all care delivered.
  • Plan and coordinate discharges, inform patient/MD, record disposition, and complete discharge paperwork.
  • Perform all other duties as assigned.
  • Successfully pass VNA pre-hire medication test.
  • Hold current RN license in Mass. and related credentials.
  • Minimum one (1) year of nursing experience or 6 months with a BSN.
  • CPR certification required.
  • Home care, IV experience, computer skills, and BSN preferred.

Purpose of Position

Performs comprehensive nursing assessments upon patient admission, considering physiological, psychological, environmental factors, and health history to ensure safety and quality of care. Integrates data to identify problems and develop care plans aligned with case management objectives.

Schedule Details

Weekend Baylor, 30 hrs/week (Paid for 37.5), 8a-6p, 30-minute unpaid lunch, Saturdays and Sundays, one weekday 8a-6p, rotating holidays, area: Upper Cape. Sign-On Bonus Eligible!
