The Registered Nurse (RN) reports to the Director of Care and is responsible for coordinating and overseeing all aspects of resident-focused care. The RN provides direct supervision, leadership, and education to healthcare team members.
Acts as a resource and advocate for residents, families, and staff, promoting the values, goals, and philosophy of the Home.
Responsibilities
- Develop and modify care plans based on residents’ changing needs.
- Liaise with healthcare professionals, agencies, and families.
- Maintain positive relationships with staff, residents, families, and the public.
- Supervise activities on their unit and take charge as needed.
- Monitor performance and conduct evaluations for PSWs.
- Assign, evaluate, and supervise the work of PSWs.
- May be in charge of the Home as required, using leadership skills to identify and resolve issues.
- Ensure care delivery aligns with the resident Bill of Rights.
- Understand and participate in quality indicator improvements.
- Order and manage nursing supplies and equipment in accordance with policies.
- Listen to residents and families, addressing concerns appropriately.
- Conduct unit rounds at shift start and end.
- Process physician orders, administer medications and treatments per policies and standards, and document all actions.
- Maintain accurate nursing records in compliance with legislation and policies.
- Assess health status changes and communicate with SDM, POA, and interdisciplinary team.
- Complete health assessments, develop and update care plans collaboratively.
- Follow procedures for mandatory reporting and incident management.
- Perform other duties as assigned.
Qualifications
- Current Certificate of Competence with the College of Nurses of Ontario or equivalent registration in another Canadian jurisdiction.
- Basic CPR certification.
- Experience in long-term care is an asset.
- Ability to work effectively with an interdisciplinary team.
- Strong written and oral communication skills.
- Knowledge of RAI-MDS; certification preferred, or must be completed within one year.
- Demonstrated focus on person-centered care and risk management.
- Must provide a current (within six months) Vulnerable Sector Check and TB test results or chest X-ray.
- COVID-19 vaccination required.
- French or Italian language skills are assets depending on the facility location.
