RN Case Manager - Home Health Hours

• Shift: Monday - Friday, 8:00 am to 4:30 pm, no weekends or on-call
• Counties Covered: Primarily Lancaster County, with possible assistance in York County

RN Case Manager - Home Health Compensation

• Pay: $56.00 per hour
• Benefits available for full-time employees
• 401(k) with company match for full-time employees

RN Case Manager - Home Health Responsibilities

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized care plans with specific interventions to engage members
• Identify and reassess cost-effective levels of care; arrange non-medical support like housing and transportation to ensure compliance
• Coordinate care transitions as needed
• Work with physicians, social workers, discharge planners, and claims professionals to transition patients to appropriate care levels
• Engage specialty resources to achieve optimal outcomes; maintain detailed records of clinical, functional, and fiscal results; identify health promotion opportunities
• Prevent adverse events and intervene promptly to minimize poor outcomes
• Act as a patient advocate, ensuring privacy and confidentiality

RN Case Manager - Home Health Requirements

• Associate's or Bachelor's Degree in Nursing or related field
• Current unrestricted state RN license
• Ability to analyze complex medical information and make rational decisions
• Case Management certification is a plus but not required
