We are looking for an experienced, talented, and compassionate RN Case Manager for a full-time position in the Hull area.
The RN Case Manager provides direct patient care in the home, conducting assessments, developing care plans, and collaborating with physicians to ensure optimal care. The role involves care coordination and identifying additional service needs.
VACCINE REQUIREMENTS: NVNA and Hospice requires full vaccination for Covid-19 and flu as a condition of employment, with an option to request an exception.
