Hours: 40hrs
Shift: Day shift, 8:00am - 4:30pm with every 4th weekend, 2 holidays
Location: Visiting Nurse Association - Fairhaven, MA

Under the supervision of the Director, Supportive Care Services or designee, and within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physicians orders, Plan of Care and Hospice and Home Care policies and procedures.
Compensation: Pay rate will be determined based on level of experience.
