We are searching for a talented RN-Care Navigator
Hours: 40hrs
Shift: Day shift; 8:30am - 5:00pm
Location: Orthopedic Surgery – Dartmouth, MA

Compensation: Pay rate will be determined based on level of experience.
Position reports to the Medical Director of Service Line and Director/Manager/Designee. Provide consistent care coordination across the continuum, assessing the physical, psychological, and social needs of the patient. Serve as ongoing resource for patients throughout the care continuum. Organize and facilitate clinical visit needs. Assist patients and families by providing education, support, and information regarding available resources for assigned patient population within the Care Center and Southcoast community. Provide educational materials and referrals to appropriate support groups and services. Support patients and families to ensure compliance with care. Create, monitor, and improve clinical quality indicators for program evaluation and any applicable regulatory or accreditation requirements. Facilitate prospective and/or retrospective case conferences, documenting conference outcomes including treatment recommendations for each patient.
