Job summary

Intermountain Health seeks an RN Hospital Care Manager I to provide comprehensive care management across inpatient and outpatient settings. You will assess patients, develop tailored care plans, and coach self-management for improved outcomes at St.

Marys Regional Hospital, Grand Junction, CO. The role emphasizes collaboration with care teams, patients, families, and payers, with focus on timely, high-quality care and adherence to policies, while participating in data-driven improvement

Qualifications

• Current Registered Nurse (RN) license in state of practice.
• BSN from an accredited institution (degree verification required). RNs hired or promoted into this role must obtain BSN within four years.
• Clinical nursing experience in an inpatient hospital setting.
• Understanding of disease management including treatment, length of stay, barriers to care, and variation.

Responsibilities

• Assessment: Identify patients using screening criteria and conduct comprehensive evaluations.
• Care Planning: Develop, adjust, and coordinate care plans to address medical and social needs.
• Patient Support: Guide self-management using motivational interviewing and coaching.
• Education & Advocacy: Educate healthcare teams and patients, advocate for patient rights, support self-care.
• Collaboration: Work with multidisciplinary teams to ensure timely, high-quality care.
• Process Improvement: Lead initiatives to enhance care transitions and policy adherence.
• Advanced Care Planning: Provide Goals of Care education for informed decisions.
• Data Analysis & Compliance: Track key metrics, analyze trends, ensure regulatory compliance.
