Return to Practice Nurse - Royal Free and Barnet Hospital

CLINICAL RESPONSIBILITIES

Maintain professional behaviour at all times and promote a positive image of Royal Free London NHS Foundation Trust, in line with World Class Care values. Assist staff in delivering direct patient care, undertaking skilled nursing activities for which training and competency assessment have been completed.

Assist nursing staff in maintaining patients' well-being and dignity in a safe environment, respecting patient confidentiality and privacy. Understand and adhere to all relevant trust policies and procedures. Prioritise care appropriately.

Demonstrate good communication skills when dealing with patients, relatives, and the multidisciplinary team, using sensitivity and empathy, and managing conflict effectively. Contribute to effective and timely discharge planning.

Perform and report patient risk assessments (e.g., nutritional, falls, moving and handling, pressure sore risk, bedside rails). Recognise and report changes in patient condition or treatment to the registered nurse or medical staff promptly. Act as a role model for student nurses and colleagues. Report concerns regarding patient safety or well-being to the nurse in charge. Escalate incidents immediately.

Understand the principles of the Friends and Family Test and patient feedback, and how it is used to improve patient experience. Assist with ward preparation for admission, ensuring cleanliness on discharge or transfer. Orientate patients and visitors to the ward environment. Ensure compliance with infection control, isolation, and food hygiene policies. Act in accordance with the NMC Code of Conduct.

RESPONSIBILITY FOR PATIENTS

Support safeguarding principles and escalate concerns. Respect patients' privacy and dignity. Contribute to a safe, therapeutic environment. Communicate appropriately with patients and relatives. Assist with dietary needs, hygiene, and personal care. Support patient mobility and repositioning within manual handling guidelines. Monitor and record observations, including blood pressure, pulse, temperature, blood glucose, and urine tests. Record patient height, weight, and BMI.

RESPONSIBILITY FOR POLICY AND SERVICE DEVELOPMENT

Answer telephones and relay messages. Participate in audits and stay updated with practice changes. Safeguard patients' property according to Trust policy.

RESPONSIBILITY FOR FINANCIAL AND PHYSICAL RESOURCES

Maintain a clean and safe environment, alert to cross-infection risks. Assist with restocking supplies and report low stock levels. Ensure equipment is stored correctly, in working order, and report faults. Support cleaning and maintenance of equipment.

RESPONSIBILITY FOR LEADING AND MANAGING

Assist with patient admissions and discharges. Identify documentation issues and report to the registered nurse.

RESPONSIBILITY FOR INFORMATION RESOURCES

Maintain patient confidentiality and ensure safe, confidential medical records. Keep documentation clear, concise, and legible, adhering to NMC and Trust standards.
