A regional healthcare provider is seeking a Community TB Support Nurse to join their team in High Wycombe. We are looking for that Nurse to join our TB team.
We have a band 6 post for a Community TB Support Nurse, working alongside the Band 7 TB Nurse Specialist and consultant lead
This is a Monday to Friday post 8am-4pm, do not work Bank Holidays.
the post holder will be based at Wycombe hospital but will be expected to travel to to various Trust sites and visit patients as needed within the county of Buckinghamshire.
We run a corporate caseload of clients, managing their care in their home.
A valid driving licence is essential for this role as the post holder will be required to travel to various BHT venues, community centres and patient homes within Buckinghamshire.
The post holder will provide specialist Tb input supporting the service pathway within the acute, community setting and outpatient setting, using highly developed specialist practice underpinned by theoretical knowledge
The Role encompasses all aspects of individualised care for our clients.
We carry out Contact tracing, DOT /VOT under guidance from N.I.C.E
The role is varied as we run Nurse Led clinics, initiating diagnostic tests for patients and commencing patients on Active treatment and LTBI treatment.
The role is varied as we offer an inreach service to our Hospitals.
We are part of the Thames Vallet TB Network with the UKHSA.
Advice and support is provided to patients (and their carers where appropriate) including the provision of education and self-management support.
Support skills and knowledge development in community nursing and acute care.
Provide specialist clinical knowledge and expertise in their related field, to other BHT colleagues as needed.
Mentoring of support
Work in collaboration with the Team to look at new and innovative ways of providing a positive patient and staff experience. This will be demonstrated with the patient and staff experience feedback questionnaires.
Work closely and supportively with the Specialist Nurses and directly with the Department teams. This will allow facilitation to develop and improve nursing practice; improve staff morale and patient experience.
