Job summary

Sheffield Teaching Hospitals NHS Foundation Trust is seeking an experienced nurse to join the specialist Renal Transplant Team. You will support patients throughout their transplant journey, combining clinical care with education and coordination across inpatient and outpatient settings.

Under supervision of the Lead Nurse and Recipient Coordinator, you will provide specialist nursing assessment, education for patients and families, participate in ward rounds, and refer to specialist services as

Qualifications

• Specialist renal transplant nursing knowledge and experience.
• Ability to educate patients and families about post-transplant care.
• Strong coordination skills across inpatient and outpatient settings.

Responsibilities

• Provide specialist nursing assessment, advice and support to renal transplant recipients.
• Deliver education to patients and families about medicines, adherence, and self-management.
• Participate in ward rounds (RUF) and support patients during inpatient stay.
• Review patients in the outpatient setting post-discharge and support transition to the clinic pathway.
• Coordinate pre- and post-transplant care with renal transplant teams and community services.
