Sheffield Teaching Hospitals NHS Foundation Trust is seeking a Band 6 Renal Transplant Nurse at the Northern General Hospital in Sheffield. This permanent, full-time post (37.5 hours per week) includes on-call duties and a diverse range of clinical responsibilities.

You will work with the Renal Transplant Team to provide specialist nursing care, patient education and care coordination across inpatient and outpatient settings, supporting patients through pre- and post-transplant pathways.

Qualifikationen

• RNCM Part 1 registration and first degree or equivalent.
• National practice assessor training or ENB998 teaching qualification.
• Extensive experience in Renal Care.
• Experience caring for patients post renal transplant.
• Computer literate with strong written and verbal communication skills.
• Experience with clinical audit and data collection.

Aufgaben

• Provide specialist nursing support to renal transplant recipients during the immediate and early post-transplant period.
• Deliver individualized education to patients and families.
• Participate in ward rounds and support patients during inpatient stay.
• Review patients in outpatient settings post-discharge and follow through to transplant clinic pathway.
• Reinforce education on medicines, adherence and self-management.
• Identify psychosocial needs and facilitate referrals to renal psychology or renal social work.
