Qualifications

• Associate degree in any course.
• Minimum 2 years of direct adult patient care experience.
• Experience interacting with healthcare team.
• Strong computer proficiency and ability to learn EMR software.
• Exceptional communication and interpersonal skills.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions.
• Assess needs and develop individualized care plans.
• Coordinate services with clinical and community resources.
• Identify and resolve barriers to care and independence.
• Monitor progress and adjust strategies; ensure compliance.
• Navigate multiple computer systems in a fast-paced environment.
