Job summary

Cornwall Partnership NHS Foundation Trust is seeking a Virtual Ward Nurse/AHP to assess, plan, and evaluate care in patients’ homes, enabling independence and quality of life. You will work with patients, carers and the wider team, offering support and advice while adhering to policy and evidence-based practice.

You will monitor patients virtually and provide face-to-face visits when needed, mentoring students and cooperating with locality teams to ensure flexible, high-quality care.

Qualifications

• Registered Nurse Adult/AHP with current UK registration.
• Mentorship module or willingness to undertake.
• Post-registration diploma or degree preferred.
• Ability to manage the caseload and support ward managers.

Responsibilities

• Admit, assess, plan, implement, and evaluate patient care.
• Work in partnership with patients and carers to promote informed choice.
• Regularly review care delivery and documentation.
• Mentor pre-registration nurses and preceptees.
• Liaise with other agencies and professionals as appropriate.
• Provide autonomous, evidence-based care in line with policies.
