Job summary
Lancashire Teaching Hospitals NHS Foundation Trust is recruiting a Staff Nurse to join the Hospital at Home/Virtual Ward team in Central Lancashire. You will deliver hospital-level care in patients' homes or residences, working with a multidisciplinary team and supported by remote monitoring and visits.
The role requires a valid driving licence and access to a vehicle for home visits, with shifts across 7 days 8am-6pm (planned extension to 8am-8pm).
Qualifications
- NMC registration and a professional qualification are essential for practice.
- Mentorship qualification or willingness to work towards it is expected.
- Active engagement in continuing education and professional development.

Responsibilities
- Assess, plan, implement and evaluate care in patients' homes.
- Remote monitoring and telephone assessments.
- Coordinate care with junior staff and the MDT.
- Maintain accurate records and contribute to MDT discussions.
- Support learners and develop nursing practice within guidelines.
