ABL Health Ltd in Gloucestershire is recruiting a Healthy Beginnings Practitioner to deliver remote health coaching by phone and video, with occasional face-to-face sessions across the county.

You will manage a caseload covering smoking cessation, healthy weight, nutrition, physical activity and postnatal healthy lifestyles, building person-centred relationships and recording progress. You will contribute to audits, reporting and continuous improvement, and strengthen referrals.

Responsabilités

• Deliver predominantly remote health coaching via telephone and video appointments, with occasional face-to-face sessions across Gloucestershire.
• Manage a caseload and provide personalised support across smoking cessation, healthy weight, nutrition, physical activity and postnatal healthy lifestyles.
• Use motivational interviewing and behaviour change techniques to support sustainable outcomes.
• Maintain records, contribute to audits, reporting and continuous service improvement, and promote the service to partners and referrers.
