Job Summary:

The Team Leader is a key member of the support team, responsible for addressing the practical, physical, and emotional needs of the individuals supported within the service.

They will design individual support programmes and coordinate their delivery by Support Workers and themselves, using a key worker system.

About The Role
Key Accountabilities
  • Plan, coordinate, and facilitate the safe transition of new individuals into the service and community where necessary.
  • Liaise with families, multi-disciplinary teams, community groups, and voluntary bodies to support the individuals' needs.
  • Develop and implement support programmes to enhance independence and capabilities.
  • Conduct ongoing assessments and reviews using appropriate models.
  • Support physical, emotional, and personal needs, ensuring respect, privacy, dignity, and independence.
  • Manage medication storage and administration according to policies and legislation.
  • Provide leadership and supervision to Support Workers, monitor attendance, and participate in HR procedures.
  • Engage in learning and development, and support personal development for themselves and staff.
  • Maintain accurate records and produce reports as per organizational and statutory requirements.
  • Assist in regulatory compliance with relevant bodies.
Requirements of the Role
  • Participate in appraisals and staff reviews.
  • Model company values and behaviors.
  • Engage in supervision sessions.
  • Perform duties in accordance with policies and regulations.
  • Take on additional duties as required.
  • Work hours to meet service needs, including sleep-ins with additional pay of £45 per night.

Note: Duties and locations may change to meet organizational needs.

Qualifications and Experience
  • At least 2 years’ experience in health and social care, with a commitment to professional qualification, OR
  • Qualifications in Social Work, Allied Health Professions, or Level 5 Leadership and Management in Health and Social Care.
  • OR 1 year paid experience with a commitment to qualifications and a Psychology Degree.

Candidates should be willing to complete Level 2 Safe and Effective Care Practice Diploma or be registered with NISCC/NMC or an Allied Health Professional.

Must have a valid driving license and access to a vehicle, or demonstrate how mobility needs are met if disabled.
