Job Description

Job responsibilities

You'll lead the Rehabilitation and Wellbeing Team in delivering high quality palliative rehabilitation and wellbeing support to patients and carers. This involves a holistic approach to help people live as well as they can for as long as they can, across inpatient, outpatient and community settings. You’ll work with colleagues from a range of disciplines and advise the hospice’s hospital-based team, sharing expertise to deliver the best care.

Key Responsibilities

  • Clinical Practice & Leadership: Lead and champion the service; build strong partnerships with colleagues across services; provide evidenced-based clinical expertise and leadership; support team growth and ensure staff confidence.
  • Develop personalised care plans to support mobility, independence, dignity and social networks.
  • Lead by example with compassionate, skilled care; stay curious and evidence-based; shape and grow the service; advocate for patients and ensure dignity before, during and after death.
  • Engage with home visits and liaise with the integrated neighbourhood team; help patients set meaningful goals and navigate their journey.
  • Contribute to funding cases by sharing impact and outcomes; recruit a values-aligned team; participate in locality and network forums.
  • Quality, Governance & Resources: Ensure equipment and records are up to date; respond to incidents with learning and improvement; make best use of resources to reach more people.
  • Communication: Shape an open team culture; ensure joined-up, compassionate care; maintain clear and timely records; utilise digital tools; communicate with honesty and clarity.
  • Ethics & Inclusion: Uphold anti-discriminatory practice; promote inclusion, fairness and respect for all patients and staff.
  • Education & Training: Encourage learning; contribute to training plans; ensure mandatory training is up to date; support students and visiting professionals.

Other responsibilities

Person Specification

Experience

Essential

  • Evidence of working in varied settings with people with complex physical needs
  • Experience delivering teaching/training
  • Experience in leadership and line management
  • Experience working with multiple stakeholders
  • Experience of change management and clinical audit

Desirable

  • Specialist palliative care experience
  • In-depth experience of legislation and policy around mental capacity and specialist provision

Skills, Knowledge & Abilities

  • Policy shaping and setting standards; health and safety awareness
  • Understanding of grief and end-of-life care systems and laws
  • Strong communication skills; digitally capable; able to work within an MDT
  • Commitment to holistic, person-centred care
