The Rehabilitation Support Worker role will function as part of the new and exciting Recovery At Home team by providing care, support and rehabilitation within a community setting to facilitate discharges from hospital or prevent admission. The post holder will undertake delegated tasks following a care plan and report to the Team Lead. Support will be provided to develop skills and abilities to work with this client group effectively.

We seek dynamic, caring individuals to play a key role in our Discharge to Assess services in Surrey Downs. Candidates should have excellent organizational and time management skills and be passionate about delivering holistic, patient-centered care. This role offers an opportunity for proactive individuals eager to advance their careers within a forward-thinking team.

Part-time opportunities are available for morning shift applicants.

Main duties of the job

Support the 'Home First' approach, advocating for patients to be supported at home for ongoing assessments, reducing unnecessary hospital stays which impact outcomes negatively. The @home service spans organizational boundaries, providing rapid, short-term health and care support to prevent emergency hospital admissions and support safe discharge, working closely with health and social care services. The Recovery AtHome team will be integral to Discharge to Assess, offering timely care and support for hospital discharge and subsequent assessment. The service operates from 8 am to 8 pm, 7 days a week, requiring flexible staffing.

Job responsibilities

Main Responsibilities and Duties

  • Deliver clinical and therapeutic care as per care plan.
  • Perform patient assessments under supervision and delegation.
  • Refer to and coordinate with other professionals as needed.
  • Provide clear handovers to team members.
  • Act as a link worker for specific roles (e.g., tissue viability, infection control).
  • Provide health education tailored to clinical needs.
  • Support patients in activities of daily living, promoting independence and dignity.

  • Use risk assessment tools to ensure safety.
  • Perform clinical skills such as vital signs measurement, blood tests, medication administration, and personal care.
