The Rehabilitation Support Worker role is part of the new Recovery At Home team, providing care, support, and rehabilitation in a community setting to facilitate hospital discharges or prevent admissions. The post holder will follow care plans, undertake delegated tasks, and report to the Team Lead. This role offers development opportunities to enhance skills working with this client group.
We seek dynamic, caring individuals to drive our Discharge to Assess services in Surrey Downs. Candidates should have excellent organizational and time management skills and a passion for holistic, patient-centered care. We value innovation and a team-oriented culture. Part-time morning shifts are available.
Home First promotes the ethos of "why not home, why not today?" Supporting patients to remain at home for ongoing assessments is preferred, with alternative pathways available if necessary. Prolonged hospital stays negatively impact outcomes, and enabling patients to continue their lives at home benefits their long-term wellbeing.
The @home service operates across organizational boundaries, providing rapid, short-term health and care support to prevent emergency hospital admissions and support medically stable patients returning home. It spans acute and community settings, ensuring coordinated care. The Recovery At Home team is integral to Discharge to Assess, facilitating timely discharge and ongoing assessment, working closely with health and social care services.
The service operates 7 days a week, from 8 am to 8 pm, with staff working flexibly across the full week. The role will evolve through acquiring additional skills and competencies, supported by the team.
