There is an expectation that this post will cover a 5 over 7 working pattern.
An exciting opportunity has arisen for Major Trauma Rehabilitation Coordinators to join the Major Trauma Hyperacute Service based at the Queen Elizabeth University Hospital (QEUH), Glasgow. We are seeking to appoint passionate, person centred and highly motivated clinicians to lead co‑ordination of the rehabilitation pathways for all patients admitted under the Major Trauma Hyperacute Service. The QEUH is one of four Major Trauma Centres in Scotland, representing the West of Scotland, which will provide all aspects of trauma care, from the point of injury to rehabilitation and into the community.
The post holder(s) will work with the hyperacute multidisciplinary team, acting as the single point of contact for patients and families, ensuring equity and consistency in rehabilitation provision and co‑ordinating the patient journey for major trauma patients under the care of the hyperacute team.
The post holder(s) will be responsible for developing and updating written rehabilitation plans for Major Trauma hyperacute patients from the point of admission to discharge, offering the post holder(s) the opportunity to be a part of the journey – working closely with both patients and the multidisciplinary team to identify and monitor patient‑centred rehabilitation goals and ensure achievement of the Scottish Trauma Network key performance indicators.
The post holder(s) will also be responsible for the repatriation of Major Trauma hyperacute patients to hospitals or Specialist Rehabilitation Units closer to their home, linking in with other coordinators across Scotland and beyond to ensure equity of service delivery and development.
It is essential that you hold a Degree/Diploma in a relevant profession and are registered with the HCPC. The successful applicant will have senior clinical experience working within a rehabilitation speciality and will have the ability to lead in the provision of clinical expertise, offering advice and support to patients, their carers and other health care professionals.
