Do you enjoy completing prescription rehabilitation with both Physiotherapists and Occupational Therapists, managing a complex caseload during inpatient stays?

Do you enjoy participating in and running groups such as exercise classes and mindfulness sessions?

Due to progression, we have a fantastic part-time Band 3 Rehabilitation Assistant opportunity (30 hours). We offer flexible working arrangements to support work-life balance and personal needs.

The role is based within Oake Meadows, a care home team within walking distance of Taunton city centre. Our ethos is to enable individuals to be as independent as possible.

We support individuals' ongoing rehabilitation and post-discharge needs from the acute hospital, assisting them through subsequent stages of their journey into their home environment. This role collaborates with ongoing services to help individuals maximize their potential in all aspects of occupations meaningful to them.

Join our friendly therapy team, which includes Physiotherapists, Occupational Therapists, and Rehabilitation Assistants, alongside the wider MDT of Discharge Facilitators, Older Persons Mental Health staff, Adult Social Care, and care home staff, to gain invaluable community and rehabilitation skills.

Main duties of the job

• Provide effective support to Occupational Therapists, Physiotherapists, and team members as an integral part of the Intermediate Care Service.
• Support patients undertaking therapy and rehabilitation programmes to help them achieve their optimal level of independence and quality of life, under the guidance of team therapists.
• Work with patients in various community locations, including community hospitals, care home rehabilitation units, and in their own homes, under the supervision of rehabilitation professionals.
• The post requires working hours over 7 days, including weekends and bank holidays, to meet service needs. A UK driving licence, business insurance, and access to a car are essential.
