Job summary

HCRG Care Group is seeking a compassionate Rehabilitation Assistant to join the Interdisciplinary Team at Swindon Intermediate Care Centre on a full-time basis (37.5 hours per week). The role includes a 6-month rotational placement between Swindon Intermediate Care Centre and the Community Stroke Team, offering exposure to inpatient and community settings.

Join our dedicated Interdisciplinary Team and play a key role in supporting individuals to regain confidence, improve their quality of life, and achieve their rehabilitation goals following illness, injury, or stroke. We are seeking a compassionate and motivated Rehabilitation Assistant to join us on a full-time basis (37.5 hours per week). Based at Swindon Intermediate Care Centre, you will work alongside therapists and other healthcare professionals to support the assessment, planning, and delivery of personalised rehabilitation programmes. The role includes a 6-month rotational placement between Swindon Intermediate Care Centre and the Community Stroke Team, providing a varied and rewarding opportunity to develop your skills across both inpatient and community settings. What You'll Be Doing
  • Supporting Physiotherapists, Occupational Therapists, and Speech and Language Therapists in delivering high-quality rehabilitation programmes across the Intermediate Care and Community Stroke services.
  • Managing a delegated caseload, delivering therapeutic interventions and treatments in line with individual care plans and under the guidance of registered therapists.
  • Helping patients maximise their physical, cognitive, and social independence, supporting them to achieve their rehabilitation goals.
  • Implementing treatment programmes and rehabilitation activities for patients recovering from illness, injury, or stroke.
  • Working closely with patients, families, and carers to ensure they are actively involved in decisions about care and rehabilitation.
  • Contributing to preventing unnecessary hospital admissions and supporting timely discharge from inpatient services where appropriate.
  • Maintaining accurate records and assist with administrative duties, including preparing correspondence, updating documentation, and ordering supplies to support the smooth running of the service.
  • Collaborating effectively with the wider multidisciplinary team to deliver person-centred, outcome-focused care.
  • Participating in a shift rota, including weekend working, to ensure continuity of service delivery.
What You'll Bring
  • Essential GCSEs (or equivalent) in English and Maths, with strong literacy and numeracy skills.
  • Confidence using IT systems, including Microsoft Word, Outlook, and internet-based applications.
  • A relevant NVQ qualification or equivalent experience in a health, social care, or rehabilitation setting.
  • Knowledge and understanding of stroke rehabilitation and the rehabilitation process.
  • Experience supporting individuals through rehabilitation, recovery, or reablement programmes.
  • Experience working within the NHS or NHS-commissioned services.
  • Previous experience in a relevant role, such as Rehabilitation Assistant, Physiotherapy Assistant, Occupational Therapy Assistant, Speech and Language Therapy Assistant, or Carer.
  • Experience working in community-based services or supporting patients in their own homes.
  • Knowledge of safe moving and handling techniques, ideally within a stroke rehabilitation setting.
  • Ability to work independently, use initiative, and take a proactive approach to patient care.
  • Strong organisational skills, with the ability to manage and prioritise your own workload.
  • Excellent interpersonal and communication skills, including the ability to support people with communication difficulties.
  • A caring, empathetic, and compassionate approach to working with patients, families, and carers.
  • The ability to problem-solve, adapt to changing situations, and respond flexibly to patient needs.
  • A full UK driving licence and access to a vehicle insured for business use.
  • DesirableFoundation Degree or equivalent qualification in a relevant subject area.
  • Experience working at Band 4 level within a healthcare or rehabilitation service.
