Secondment opportunity for 12 months (maternity leave cover). An exciting opportunity has arisen to join the Doncaster Community Stroke Rehabilitation Team, which comprises Occupational Therapists, Physiotherapists, Nursing staff, and Rehabilitation Assistants. This service covers the Early Supported Discharge and Community Rehabilitation Service.
The Rehabilitation Assistant will support qualified professionals and contribute to the assessment, implementation, and delivery of care within patients' homes.
Report patient progress and any changes in condition to the appropriate person.
Ensure accurate documentation of care within electronic record systems.
Develop clinical skills through competency-based training and maintain skills via regular re-assessment.
Demonstrate sensitivity to patients and carers by respecting privacy and dignity, maintaining professional boundaries.
Build rapport with patients/service users and encourage participation in rehabilitation and healthcare programmes.
Work independently within the community following a period of supervised practice.
