Overview

We have an exciting opportunity for a highly motivated Practice Nurse based at Tredegar Medical Practice.

The post holder will be a highly skilled and experienced Practice Nurse with a sound knowledge of primary care nursing and be able to provide clinical mentorship to other registered nurses and Health Care Support Workers.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Responsibilities

The post holder will contribute to the delivery of high quality nursing care, providing specialist advice and clinical services to patients.

Responsible for the assessment, development, implementation and evaluation of patient needs. The post holder will assist in the implementation of Health care policies and will promote evidence based practice and act as an accountable role model.

The post holder will support the Senior Nurses ensuring the delivery of Healthcare services to the patients within primary and community care settings. They will work clinically alongside members of the Nursing Team and wider multi-disciplinary teams to ensure the provision of effective, efficient, appropriate quality evidence based care to patients.
