Registered Nurse (RN) – Complex Community Care (Shropshire)

Location: Shropshire

Client: Female, 43 years old, living independently with her dog

Employment Type: Flexible hours available

About The Role

At Harmony Healthcare, we are proud to deliver exceptional, relationship-centred care to clients in the community. We are seeking an experienced and compassionate Registered Nurse (RN) to join our team, supporting a valued client who is living independently with her beloved dog in Shropshire.

Our client, a 43-year-old woman, requires 24-hour nursing support to manage her complex healthcare needs, including Type 1 Diabetes, wound care, and emotional wellbeing. This role offers a unique opportunity for a Nurse who is passionate about truly making a difference, offering stability, independence, and a trusted clinical presence within a home environment.

Key Responsibilities

Clinical Oversight

Manage and monitor diabetes care, delivering a complex insulin regimen (Fiasp, Tresiba, and PRN Novorapid) according to detailed blood glucose monitoring protocols.

Medication Management

Support full medication administration, ensuring compliance and titration based on blood glucose levels. Monitor post-meal insulin administration and PRN interventions.

Wound And Skin Integrity Management

Work collaboratively with District Nurses to support the ongoing treatment of necrotic ulcers, skin lesions, and diabetic foot care needs.

Behavioural And Emotional Support

Apply trauma-informed care principles and Positive Behavioural Support (PBS) approaches, responding effectively to emotional distress, refusal to engage, and challenging behaviours.

Risk Management

Proactively identify and mitigate clinical, emotional, and environmental risks, supporting the client’s safety and wellbeing in her home.

Team Collaboration

Operate as part of a multidisciplinary care team, coordinating with Healthcare Assistants, District Nurses, Community Mental Health services, and Clinical Case Managers.

Home Environment Management

Deliver nursing care within a specially adapted home environment, including the use of a hoist, hospital bed, Nimbus mattress, and safety adaptations.

Candidate Requirements

Essential

Active NMC PIN registration.

Proven experience supporting adults with complex healthcare needs in a community setting.

Confidence in working independently while contributing to a wider multidisciplinary team.

Excellent communication skills and accuracy in clinical record keeping.

Comfortable working in a home with a dog.

Desirable

Experience supporting individuals with Emotionally Unstable Personality Disorder (EUPD).

Advanced understanding of diabetes management and wound care protocols.

Previous trauma-informed care training or experience.

A passion for delivering highly personalised, one-to-one nursing care.
