East Bay Community Action Program (EBCAP) is hiring for a Registered Nurse, RN, to work full-time day shift at our health center located in Newport, RI!

Responsibilities
- Provide ongoing health assessment (identify health issues, behaviors, needs, barriers) and all other assessments which are appropriate for the nursing scope of practice. Ensure that all screening tests are up-to-date with results recorded in the electronic medical record (EMR).
- Assist assigned primary care physician and/or advanced practice registered nurse, and/or physician assistant in all aspects of office-based primary care, behavioral health care, maternity care, etc.
- Provide a wide variety of skilled office-based nursing services in support of patient treatment plans and documented orders by physicians and/or advanced practice registered nurse, and/or physician assistant, within scope of license and experience.
- Collaborate and regularly liaise with internal/external providers, internal/external pharmacies, internal/external labs, and community agencies, such hospitals, long-term care facilities, and home health representatives based on consumer’s health and wellness needs.
- Perform appropriate follow-up on transitions of care including medication reconciliation.
- Record and administer medications prescribed by the team physician/psychiatrist; evaluate and chart medication effectiveness, complications, and side effects. Complete Prior Authorization procedures based on patient’s insurance requirements as needed.
- Provide back-up or fill-in for other nurses within treatment team, pod, location, etc. Coordinate any clinical “down time” with Supervisor/Manager to maximize the efficiency and support of the nursing care provided to clients, regardless of primary assignment.
Qualifications
- Minimum of an Associate's Degree in Nursing or equivalent; Bachelor's degree preferred.
- Licensed as a Registered Nurse (RN) by the RI Department of Health
- 1 year of clinical nursing experience preferred, but not required
- Experience with a computer based EHR
- Experience in a community health or primary care setting preferred but not required
- Bilingual capability preferred not required
