Registered Nurse, Palliative Support

Are you a dedicated and empathetic nurse passionate about making a difference in people's lives? UHCW is seeking a Community Nurse to join our Palliative Care Team, delivering high-quality, person-centred care to patients with life-limiting conditions in their homes.

Your role:

• Provide holistic palliative care in the community
• Work closely with a range of professionals to ensure seamless, coordinated care

Please note this post is not suitable for applicants who are required to undertake the OSCE training programme. Applicants who do not have current valid UK NMC registration will not be considered for this post. You must also have post-preceptorship experience.

Successful applicants will be required to provide an immunisation and vaccination report, from either their current UK occupational health provider or their UK GP.

Main duties of the job

• To work under the direction of the senior clinician/ coordinator on shift.
• To sign in/out all equipment in logbook when taking from the store cupboard i.e. syringe drivers.
• To clean & document return of equipment in store cupboard.
• To safely dispose of sharps bins/clinical waste as per policy.
• To ensure patient deaths are recorded in required documentation book.
• Safely administer medication to provide symptom control to palliative patients as per Trust policy
• To liaise with GPs and other members of the multidisciplinary team to always ensure seamless care, including ordering medication, authorisation etc
• To liaise with pharmacies to ensure medications are delivered in a timely manner and ensuring patients always have medications
• Holistic assessment of patients' needs, recording those wishes that are important to them.
• To complete the Continuing Health Care team Fastrack assessment to ensure funding or further referral.
• To provide regular reassessment of palliative support patients to ensure needs are being met.
• Documentation and care planning, including use of Individual Plan of Care of the dying person and IPOS (Initial and weekly).
• Escalate any concerns to senior nurse

Job responsibilities

• Assessment of pressure area care and skin checks on initial assessment and subsequent visits. Relevant documentation to be completed on patient record.
• Continence Care for patients known to the palliative support team including ordering of continence products and catheter insertion procedures.
• Assessment of appropriate equipment and ordering of such equipment.
• Wound care- Implementation of wound care plans, including photographs and documentation to be uploaded onto patient record. To escalate and refer patient to Tissue Viability to support where required.
• Emotional support for patients and those important to them including accessing outside agencies and services when required.
• Assist with the day to day running of the team.
• To complete inbound referrals via EMIS to relevant service as required.
• To coordinate the palliative support team as required if no band 6 nurse available. Be aware of own limitations and gain support from band 7 on shift where required.
• To attend multi-disciplinary team meetings on a regular basis and daily handovers.
• To report incidents in a timely manner via Datix and escalate as required.
• To understand and follow the safeguarding process with any concerns.
