Overview

Community Nurse – Palliative Care Team. Are you a dedicated and empathetic nurse passionate about making a difference in people’s lives? UHCW is seeking a Community Nurse to join our Palliative Care Team, delivering high-quality, person-centred care to patients with life-limiting conditions in their homes.

Responsibilities
  • Provide holistic palliative care in the community
  • Work closely with a range of professionals to ensure seamless, coordinated care
  • Support patients and families with compassion and dignity
  • To work under the direction of the senior clinician/coordinator on shift
  • To sign in/out all equipment in logbook when taking from the store cupboard i.e. syringe drivers
  • To clean & document return of equipment in store cupboard
  • To safely dispose of sharps bins/clinical waste as per policy
  • To ensure patient deaths are recorded in required documentation book
  • Safely administer medication to provide symptom control to palliative patients as per Trust policy
  • To liaise with GPs and other members of the multidisciplinary team to always ensure seamless care, including ordering medication, authorisation etc
  • To liaise with pharmacies to ensure medications are delivered in a timely manner and ensuring patients always have medications
  • Holistic assessment of patients’ needs, recording those wishes that are important to them
  • To complete the Continuing Health Care team Fastrack assessment to ensure funding or further referral
  • To provide regular reassessment of palliative support patients to ensure needs are being met
  • Documentation and care planning, including use of Individual Plan of Care of the dying person and IPOS (Initial and weekly)
  • Escalate any concerns to senior nurse
Requirements
  • Current valid UK NMC registration
  • Post-preceptorship experience
  • Must not be required to undertake the OSCE training programme for this post
  • Full UK driving licence and access to a vehicle
  • Successful applicants will be required to provide an immunisation and vaccination report from a UK occupational health provider or UK GP
