Shift Pattern
- 3 × 12.5‑hour shifts (7:00 am–7:30 pm) per week for 3 weeks.
- 4 × 12.5‑hour shifts (7:00 am–7:30 pm) on the 4th week.
- One shift will be protected for mandatory training, supervision and administration on site.
- 2 weekends in every 6‑week roster (both Saturday and Sunday).
- One set of 3‑night shifts every 12‑week roster (7:00 pm–7:30 am), 2 per annum on weekends.

Responsibilities
- Deliver high‑quality clinical care within the Integrated Healthcare Service, including acute & primary care, long‑term condition management, enhanced assessment and diagnostic intervention services, and health promotion activities.
- Carry a radio and be competent to lead in medical emergency responses.
- Provide robust expert clinical skills with evidence‑based practice to ensure a proactive, outcome‑driven approach on a day‑to‑day basis.
- Support the development of evidence‑based practice and promote research as appropriate.
- Participate in managerial and professional clinical supervision, training, annual appraisal and ensure it becomes integral to team practice.
- Implement patient‑centred care planning tools, long‑term condition registers and consistent delivery across all prisons within the service.
- Work collaboratively with the Clinical Lead, GPs and other clinicians to meet and review service needs.
- Be part of skilled multidisciplinary teams comprising all healthcare staff, strategic partners and prison management to manage a single referral pathway.
- Undertake training to maximise uptake of screening, health promotion and prevention activity, including vaccinations.
- Ensure a named care coordinator is allocated for every patient with complex needs, providing a proactive, evidence‑based approach to clinical interventions, health promotion/prevention activity and management of acute and long‑term conditions.
- Develop and review single integrated care plans and comprehensive risk assessments in a needs‑led review cycle.
- Undertake training in evidence‑based CBT approaches to enhance consistency and quality of care delivery.
- Support patients to manage their health at every stage, from oral health to long‑term conditions.
- Embed health promotion into every aspect of the service.
- Liaise with senior Trust personnel to share expertise, promote ideas and ensure the service works collaboratively with other Directorates.
- Work positively and effectively in a difficult and often hostile environment.
Qualifications & Requirements
- Registered Nurse or Paramedic in good standing, with Band 5 experience preferred.
- National Security Vetting; all applicants must be willing to undertake vetting as part of pre‑employment checks.
- Proof of right to work and photographic ID.
- Proof of address documentation (5 years address history required; details provided if less).
- Non‑UK passport holders must provide correct documentation (right to work in the UK) and a Home Office Share code.
- UK Passport holders who have lived abroad for more than six months in the last three years must provide a certificate of good conduct or an overseas police check in English.
- Non‑UK Passport holders with less than five years UK address history must provide a Police Certificate in English.
