Job Description:
Primary Responsibilities:
- Promotes Community Hospice as the preferred provider of hospice care in our service area with ability to “sell” the benefits of hospice care, resolving patient/family objections.
- Provides information about Community Hospice services and philosophy to patients, caregivers, physicians, LTC administrators, and the community.
- Implements Medicare Local Coverage Determination to assess patients for Hospice eligibility.
- Collects and communicates appropriate information to/from admission physician to determine hospice eligibility.
- Performs a physical assessment, identifies immediate needs, including spiritual, functional, psychosocial, and financial needs.
- Applies appropriate nursing interventions and documents concisely in electronic medical record.
- Facilitates Admission to Community Hospice.
- Provides emotional support to patients and families/caregivers with compassion and empathy.
- Maintains confidentiality of patients and families in compliance with HIPAA.
- Develops patient's initial plan of care based on immediate needs and obtains physician orders for medications, DME, and treatments.
- Demonstrates knowledge of medications for end-of-life care, side effects, and interactions.
- Assesses, communicates, and obtains orders to ensure patient comfort.
- Coordinates and supports changes in level of patient care and facilitates transfers based on Medicare Conditions of Participation.
- Adheres to professional ethics in service delivery.
- Engages in professional development related to end-of-life care and relevant regulations.
- Provides consultation on Advance Directives to patients, families, and staff.
- Evaluates team members and provides feedback to management.
- Mentors and educates new staff members.
- Builds relationships with physicians, social workers, discharge planners, and community agencies to facilitate patient admission and placement.
- Educates the medical community on hospice referrals and benefits of early referrals.
- Represents Community Hospice professionally at all times.
Required Licenses/Certifications:
- Registered Nurse, current licensure in Florida.
- CPR certification, current.
- Valid Florida driver’s license with acceptable driving record; reliable transportation.
- Level II AHCA background screen.
- CHPN certification preferred.
Education/Experience:
- Diploma or associate degree in nursing, or Bachelor’s degree in nursing.
- Minimum of two years hospital or LTC experience; ICU or ER experience preferred.
