Job Description:

Primary Responsibilities:

  • Promotes Community Hospice as the preferred provider of hospice care in our service area with ability to “sell” the benefits of hospice care, resolving patient/family objections.
  • Provides information about Community Hospice services and philosophy to patients, caregivers, physicians, LTC administrators, and the community.
  • Implements Medicare Local Coverage Determination to assess patients for Hospice eligibility.
  • Collects and communicates appropriate information to/from admission physician to determine hospice eligibility.
  • Performs a physical assessment, identifies immediate needs, including spiritual, functional, psychosocial, and financial needs.
  • Applies appropriate nursing interventions and documents concisely in electronic medical record.
  • Facilitates Admission to Community Hospice.
  • Provides emotional support to patients and families/caregivers with compassion and empathy.
  • Maintains confidentiality of patients and families in compliance with HIPAA.
  • Develops patient's initial plan of care based on immediate needs and obtains physician orders for medications, DME, and treatments.
  • Demonstrates knowledge of medications for end-of-life care, side effects, and interactions.
  • Assesses, communicates, and obtains orders to ensure patient comfort.
  • Coordinates and supports changes in level of patient care and facilitates transfers based on Medicare Conditions of Participation.
  • Adheres to professional ethics in service delivery.
  • Engages in professional development related to end-of-life care and relevant regulations.
  • Provides consultation on Advance Directives to patients, families, and staff.
  • Evaluates team members and provides feedback to management.
  • Mentors and educates new staff members.
  • Builds relationships with physicians, social workers, discharge planners, and community agencies to facilitate patient admission and placement.
  • Educates the medical community on hospice referrals and benefits of early referrals.
  • Represents Community Hospice professionally at all times.

Required Licenses/Certifications:

  • Registered Nurse, current licensure in Florida.
  • CPR certification, current.
  • Valid Florida driver’s license with acceptable driving record; reliable transportation.
  • Level II AHCA background screen.
  • CHPN certification preferred.

Education/Experience:

  • Diploma or associate degree in nursing, or Bachelor’s degree in nursing.
  • Minimum of two years hospital or LTC experience; ICU or ER experience preferred.
