We have an exciting opportunity within the Community Ready and Reablement Unit for a band 5 registered nurse. This unit is part of the Trust's 'One step closer to home' philosophy. We are looking to appoint an enthusiastic and hard-working Band 5 Registered Nurse to work with the multi-disciplinary team on the Community Ready Unit. This new post is an exciting opportunity to support patients awaiting discharge.
The Community Ready Unit offers a relaxed environment for patients who are community ready and may be awaiting various community support. The successful candidate will work as part of our Transfer of Care Hub to support patients who are community ready to continue their rehabilitation potential. This unit opened in January 2024, and we are continually striving to improve the quality of the service we provide. The post holder would be expected to work a rotation of days, nights, and weekends to meet changing service needs. We are looking for someone who is enthusiastic, creative, and positive with outstanding communication skills.
