A unique opportunity to lead. You will play a pivotal role in coordinating the care of our residents, including liaising directly with GPs and other providers. This role operates within a supportive, multi-disciplinary team that values listening to and supporting all members. It offers a chance to build meaningful, long-term relationships with residents and take a holistic approach to their physical and mental health.
Responsibilities
- Coordinate the care of residents and liaise directly with GPs and other providers.
- Work with a close-knit, multi-disciplinary team to ensure the best, safest decisions are made.
- Spend time building relationships with residents to support holistic physical and mental health care.
- Identify opportunities to improve care and collaborate with the team to implement changes when appropriate.
- Maintain and enhance clinical skills through training and learning opportunities across a range of needs (e.g., dementia care, medical emergencies, wound care, diabetes management, stroke care).
Qualifications
- NMC registration – either RGN or RMN.
- Demonstrable post-registration experience in a similar setting.
- Preferably experience within a care home or elderly setting.
- A commitment to delivering high quality care.
- Good communication and organisational skills.
- Passion and empathy with a desire to make a difference in the lives of residents.
