Position Summary

Baptist Health Brookwood Hospital

Position Summary: Administers patient care in an inpatient or observation setting where an acute care patient receives active treatment for injuries, illnesses, medical conditions, or post-intervention assessments.

Responsibilities

Essential Functions:

  1. Assess patient needs and develop/revise individualized care plans based on patient responses and progress towards expected outcomes.
  2. Respect diversity by building respectful relationships with team members and patients.
  3. Act as a patient and family advocate.
  4. Provide age-appropriate care, demonstrating necessary knowledge and skills for the patient’s age group.
  5. Serve as preceptor, charge nurse, educator, or nurse clinician as required.
  6. Collaborate with medical staff and interdisciplinary teams for effective planning and management.
  7. Model professional behavior and support hospital and nursing goals.
  8. Understand growth and development principles, assess patient status, and interpret information to meet age-specific needs.
  9. Coordinate care and delegate tasks appropriately to team members.
  10. Document patient care accurately, including assessments, education, treatments, and safety measures, meeting regulatory standards.
  11. Maintain competency in nursing skills and equipment use.
  12. Prioritize care based on Evidence-Based Practice Standards.
  13. Identify and resolve problems effectively.

Hospital Care at Home (Program Functions – Patient Care Hub)

Patient Care Hub: Provides virtual hospital care using TeleHealth, assessing and revising care plans, and collaborating with in-home staff to support patient needs.

In-home Nursing:

Delivers direct care in patients' homes, assessing needs, collaborating with virtual teams, and managing urgent or emergent situations. Capable of using virtual technology for remote visits and handling logistical tasks like transportation of items and medications.

Qualifications

Education/Training: Graduate from an approved nursing program and meet all competency requirements.

Licensure/Certification: Current Alabama RN license or valid multistate license, BLS certification, and additional certifications like ACLS, NRP, PALS, TNCC as required. Valid driver’s license and reliable transportation necessary for home nursing duties.

Experience: Minimum 1-year nursing experience; 2 years for Hospital Care at Home roles, unless otherwise approved.
