Position Summary

Baptist Health Brookwood Hospital

Position Summary: Administers patient care in a hospital or inpatient/observation setting where an acute care patient receives active treatment for an injury, illness, medical condition, or post-intervention with assessment.

Responsibilities

Essential Functions:

  1. Assesses patient needs and develops/revises individualized care plans based on patient responses. Evaluates patient progress toward expected outcomes.
  2. Respects diversity by fostering respectful relationships with team members and patients.
  3. Acts as a patient and family advocate.
  4. Provides age-appropriate care, demonstrating necessary knowledge and skills.
  5. Serves as preceptor, charge nurse, educator, or clinician as needed.
  6. Collaborates with medical staff and interdisciplinary teams for effective planning and management.
  7. Models professional behavior and supports hospital and nursing goals.
  8. Applies principles of growth and development, assesses patient data, and interprets information to meet age-specific needs.
  9. Coordinates care and delegates tasks appropriately.
  10. Documents patient care accurately, including assessments, education, medications, treatments, and safety measures.
  11. Demonstrates competency in nursing skills and unit-specific equipment.
  12. Prioritizes patient care based on Evidence-Based Practice Standards.
  13. Identifies and resolves problems effectively.
  14. Delegates tasks considering patient needs and team member capabilities.

Hospital Care at Home Program
Patient Care Hub (Virtual Hospital)

Administers patient care remotely using TeleHealth technology, assessing needs and developing care plans, collaborating with in-home staff, and providing virtual visits.

In-home Nursing

Provides direct care in patients' homes, assessing needs, traveling within designated regions, collaborating with virtual teams, and managing urgent/emergent issues. Capable of using virtual technology for remote assessments and delivering items as needed.

Qualifications

Education/Training: Graduate of an approved nursing school. Meets all competency requirements.

Licensure/Certification: Current Alabama RN license or valid eNLC multistate license, BLS certification. Additional certifications like ACLS, NRP, PALS, TNCC required for specific units. Valid driver’s license and reliable transportation needed for in-home nursing.

Experience: Minimum 1 year of nursing experience required; 2 years for Hospital Care at Home.
