Job description
Overview

To provide high quality palliative nursing care for patients living with advancing illness and with palliative/end of life care needs, and their families, either in St Marys Hospice premises or the patients home, or in community settings. This post is subject to unsocial working conditions i.e 24 hour shift rota and/or may involve working in the patients home.

Responsibilities
  • Undertake holistic individualised patient assessments, utilising evidence-based assessment tools to assess physical, psychological, social and spiritual needs.
  • Assess the needs of family members/loved ones and liaise with Family Support Services to meet their needs.
  • Integrate OACC and other outcome measures within individualised patient assessments.
  • Deliver evidence-based palliative and supportive nursing care, maintaining the patient at the centre of care decision making, and their family as appropriate.
  • Be proactive in reviewing patient care with nursing and multi-professional team at regular intervals, including daily review meetings and weekly multi-disciplinary team meetings.
  • Review, evaluate and update nursing care plans and progress reports, ensuring accurate documentation and reporting on EMIS and on care plans.
  • Be accountable for the safe custody and administration of medicines and the accurate maintenance of records.
  • Maintain up to date knowledge of the purpose, actions and side effects of drugs.
  • Assess and report on the effect of medication and other interventions on symptom management.
  • Undertake non-pharmacological interventions to support symptom management, such as breathing and relaxation techniques.
  • Ensure patient/family confidentiality is met at all times.
  • Comply with policies, procedures and current legislation.
