Qualifications

Are you a compassionate, skilled nurse with a passion for delivering high-quality, evidence-based care? We are looking for a dedicated and proactive Registered Nurse to join our dynamic Willen at Home team to help us deliver exceptional support to patients with complex and changing needs.

Main duties of the job

What You'll Do

  • Provide continuous assessment of patients and their carers, identifying evolving needs across a varied caseload.
  • Deliver skilled nursing interventions including medication administration (via multiple routes), verification of death and assessments for care.
  • Offer psychosocial support to patients and families experiencing emotional distress, providing compassionate, evidence-based guidance.
  • Communicate effectively with multidisciplinary healthcare professionals, social workers, patients, carers, and managers across organisational boundaries.
  • Facilitate formal educational training sessions for carers and community RNs.
  • Contribute to clinical audit and research projects to drive continuous improvement and best practice.

What You'll Need:

  • Be a Registered Nurse with valid NMC registration and at least 6 months post-qualification experience.
  • Have proven experience in managing complex patient needs in a community setting.
  • Strong clinical knowledge and the ability to adapt to rapidly changing situations.
  • Excellent communication and collaboration skills across disciplines and roles.
  • Have flexibility to work across day and weekend shifts.
  • Full UK driving licence and access to a vehicle with business-use insurance (for community role).

Job responsibilities
  • Continual assessment of patients and their carers needs, when those needs are complex and change across the caseload.
  • Undertake nursing interventions (such as providing personal care to patients, administration of prescribed medication through a variety of routes, tissue viability care, ensuring infusion pumps are working correctly) consistent with evidence-based practice, transferring and applying knowledge and skills to meet individual patients needs.
  • Provide psychosocial support to patients in emotional distress and their distressed families and provide a high standard of evidence based information to them.
  • Communicating effectively with multi disciplinary health care professionals, medical and social work colleagues, patients, carers and managers across organisational boundaries.
  • Manage the workload and liaise with M D T in the absence of the Lead Nurse.
  • Participate in the delivery of formal educational training to carers and RNs in the community
  • Participate in audit/research projects in order to achieve best practice.
  • Participate in champion role work within the team to promote evidence-based practice and share learning to the wider team.
Person Specification
Experience
  • Be a Registered Nurse with valid NMC registration and at least 6 months post-qualification experience.
