Responsibilities of a Hospice RN / RN Case Manager include:
  1. Reporting directly to the Administrator, Executive Director, or Patient Care Manager and maintaining primary control and professional management of each patient.
  2. Acting as the primary liaison between the physician, patient/family, and hospice team.
  3. Providing direct patient care services to hospice patients and supportive care to the patient’s family, managing a reasonable caseload with a trusting and compassionate relationship.
  4. Empowering patient’s caregivers through education on care for their loved one.
  5. Managing and collaborating with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
  6. Evaluating patient and family needs and assisting in the development and execution of the care plan for each patient.
  7. Maintaining all assigned patient clinical records.
  8. Participating in company quality assessment and performance improvement programs.
About You

Requirements for Hospice Registered Nurse / RN Case Manager:

  1. Possess a compassionate, kind, and empathetic heart for your patients and their families.
  2. Comfortable providing care wherever the patient calls “home”.
  3. Demonstrate flexibility and composure supporting others during a crisis.
  4. Hold an RN license in the state of employment or eligibility to obtain one.
  5. Have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  6. Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
  7. Possess a current driver’s license and automobile insurance.
  8. Maintain current CPR certification.
