Responsibilities of a Hospice RN / RN Case Manager include:
- Reporting directly to the Administrator, Executive Director, or Patient Care Manager and maintaining primary control and professional management of each patient.
- Acting as the primary liaison between the physician, patient/family, and hospice team.
- Providing direct patient care services to hospice patients and supportive care to the patient’s family, managing a reasonable caseload with a trusting and compassionate relationship.
- Empowering patient’s caregivers through education on care for their loved one.
- Managing and collaborating with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
- Evaluating patient and family needs and assisting in the development and execution of the care plan for each patient.
- Maintaining all assigned patient clinical records.
- Participating in company quality assessment and performance improvement programs.
About You
Requirements for Hospice Registered Nurse / RN Case Manager:
- Possess a compassionate, kind, and empathetic heart for your patients and their families.
- Comfortable providing care wherever the patient calls “home”.
- Demonstrate flexibility and composure supporting others during a crisis.
- Hold an RN license in the state of employment or eligibility to obtain one.
- Have 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
- Additional preferred experience includes hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
- Possess a current driver’s license and automobile insurance.
- Maintain current CPR certification.
