Invicta Health, in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT), is seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together.
What is Mental Health Together?
In Kent and Medway, the community mental health transformation model has created a new service called Mental Health Together.
It is a multi-agency offer, bringing all the services and agencies supporting people with mental health needs closer together. This ensures that people receive the right care without navigating service boundaries or repeatedly sharing their story.
Mental Health Together will introduce new roles as part of a joined-up care model bridging the gap between primary care (GPs) and community mental health services (KMPT). It aims to treat people with complex mental health needs by:
- Providing person-centred, holistic support
- Removing barriers and thresholds across services
- Involving service users and carers as equal partners
- Building involvement and co-production into service design and delivery
- Delivering needs-led, personalised, trauma-informed care
Main duties of the job
The Registered Mental Health Nurse / Occupational Therapist will deliver stepped care through evidence-based interventions to promote recovery, social inclusion, and normalisation for service users with mental health problems.
They will collaborate with the MDT, CRHT, inpatient services, community teams, primary care, and the voluntary sector to support and treat service users, ensuring safe management within community settings.
The role's core purpose is to promote mental and physical health, prevent ill health, and treat mental health issues effectively, supporting strengths-based, inclusive, and co-produced care plans, especially for those with co-existing needs and marginalisation.
They will be responsible for assessment, management planning, and care delivery for allocated patients, supervising Band 5 Nurses/OTs and pre-registered students, and holding a mentoring qualification. A commitment to collaborative working and liaison with other professionals is essential.

Job responsibilities
Main Responsibilities
- Foster recovery through empowerment, using a strengths-based approach involving service users and carers in care planning, establishing supportive relationships that encourage self-determination.
- Offer evidence-based interventions and risk management strategies to help reduce and manage symptoms and high-risk behaviours during relapse episodes.
- Be proactive, innovative, and creative in delivering care to facilitate recovery.
- Engage service users and carers in relapse prevention and risk management/contingency planning, providing interventions to resolve crises and prevent hospital admissions.
- Coordinate with inpatient units to support discharge through appropriate pathways.
- Support early discharge from inpatient and CRHT teams through regular reviews.
- Deliver flexible, responsive services aligned with service user and carer needs.
- Collaborate effectively with the multidisciplinary team and other agencies.
- Provide high-quality care, solutions, and signposting to support correct pathway access for severe and enduring mental health conditions.
- Deliver care within a strengths-based model, maintaining service users in their community environments.
- Act as an educator for service users and carers, advising on mental health issues to promote health and self-management.
- Respond swiftly to urgent referrals with interventions to prevent or reduce admissions.

Person Specification
Experience
- Minimum of 2 years post-registration experience.
- Experience in community teams preferred.
Qualifications
- Registered Mental Health Nurse or Occupational Therapist (diploma or degree).
- MELPS qualification.
