Job summary

Active Care Group is seeking a Complex Care Specialist Support Worker to provide urgent, regionally-based support across assigned areas. You will work with a highly skilled team to stabilise complex care packages, support timely hospital discharges and help families feel reassured.

What you’ll do includes delivering person-centred care, assisting with daily living activities, mentoring new staff and collaborating with Care Managers and Clinical Professionals. Travel regionally is required.

Responsibilities

• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Support clients with daily living activities and promote independence.
• Build routines that improve confidence and wellbeing.
• Provide reassurance and continuity for clients and their families.
• Mentor colleagues and support new Support Workers.
• Work alongside Care Managers and Clinical Professionals.
• Ensure medication, equipment and care supplies are in place.
• Identify concerns and escalate appropriately.
