BRAMLEY HEALTH ARE LOOKING FOR A SHARP, PRO-ACTIVE INDIVIDUAL WHO ENJOYS BUILDING RELATIONSHIPS, TO JOIN THEIR TEAM AS A REFERRALS COORDINATOR ON A TEMPORARY BASIS, COVERING MATERNITY PERIOD FOR 12 MONTHS.

The role of referrals coordinator is vital to the succession of the business, and the care provided to our service users so is extremely rewarding!

Bramley Health is a specialist health and social care provider. We support men and women of working age who pose variations of mental health challenges within our homes, to maximise their independence to live a meaningful and fulfilling life.

We currently operate 8 care homes and 1 hospital around the Southeast of England, with more on the way. They vary in size, location, and services offered.

ABOUT THE ROLE

The role of the Referrals Coordinator is to support the seamless transition of individuals into our care homes and hospitals. Liaising with both internal and external stake holders and customers, you will play a pivotal role in the seamless transition of individuals right through from the point of original referral, all the way to the date of admission and beyond.

RESPONSIBILITIES

o Own and manage referrals into our care homes and hospitals, ensuring individuals are placed in the best/ most suitable home for their needs
o Reviewing and proof-reading assessment reports to ensure accuracy, in preparation for submission of funding prior to admission
o Liaise with financial stake holders ensuring all contracts and placements are accurate
o Work proactively to keep our customer database up to date and informative


KEY SKILLS

o Proven understanding/ experience within a similar role in a healthcare organisation or similarly within the NHS
o Good communication and organisation skills
o Results driven, pro-active approach to own workload
o Proficiency in Microsoft office, specifically excel
o Time management and prioritisation skills
