Job description

You will lead a fast-paced multidisciplinary referral centre, working in partnership with internal and external teams across the healthcare landscape.

Our team collaborates with primary care, acute hospital services, community hospitals, and voluntary organizations to enable people to live independently at home. The ethos is a rehabilitative, person-led approach focused on recovery and self-management, aiming to provide care close to home and avoid unnecessary hospital admissions.

The Referral Centre handles referrals for the locality Integrated Community Teams (ICTs), which include Community Nursing, Physiotherapy, Occupational Therapy, and Home First/Reablement services. We work closely with local Adult Social Care colleagues.

You will provide leadership and supervision within this dynamic environment.
