Job description

To provide a dedicated comprehensive Recovery Focused Tier 2/3 service to people with drug and alcohol use problems, their families, and careers within the organisational framework of the Bucks CC Public Health Team.

Main duties of the job
  1. To accept referrals, assess needs, and make appropriate recommendations as part of a robust 'duty system'.
  2. Provide harm reduction advice, information, and support with an emphasis on safer drug use, safer sex, HIV, Hepatitis A, B, and C, and other blood-borne infections.
  3. Be responsible for maintaining a full caseload of complex clients as the key worker.
  4. Make home visits consistent with need.
  5. Carry out comprehensive assessments, construct care packages with multidisciplinary colleagues where required, review recovery plans in a structured format, and manage onward referrals.
  6. Assist and support medical and nurse prescribers during harm reduction and prescribing clinics.
  7. Provide a range of interventions including CBT-based brief solution-focused therapy and motivational enhancement techniques.
  8. Make domiciliary visits to service users, particularly those with disabilities and family responsibilities.
  9. Retain contact with service users while they are in residential rehabilitation, including visits when required.
  10. Create and facilitate group work interventions.
  11. Demonstrate a commitment to service user involvement.
  12. Work with a range of professionals to ensure service users' needs in terms of safeguarding and mental health are met.
