The post holder will, using a range of generic and specific tools, be responsible for providing individual service user assessments, identifying, delivering, and evaluating care interventions. This will include providing direct care to service users.

The post holder will have a designated caseload of service users to provide high-quality, person-centred care, responding to their individual and diverse needs, as well as those of their significant others, delegated by the Clinical Lead or designated supervisor/manager. They may also take on a care co-ordination role for a specific group of service users.
