The Recovery Practitioner will play a vital frontline role in providing essential support to individuals recently discharged from the Swindon Recovery Team at Avon & Wiltshire Partnership NHS Trust.

Job responsibilities
  • Support the delivery of outreach services related to the Wellbeing Step-down service for people discharged from adult secondary mental health services.
  • Work closely with care coordinators and other NHS professionals within the Swindon Recovery team, supporting referrals to the Steps Programme.
  • Attend weekly meetings with the NHS Swindon Recovery team to discuss referrals.
  • Participate in handover meetings involving care coordinators and referred individuals.
  • Deliver outreach appointments according to the STEPS programme timelines.
  • Collaborate with individuals to co-create personalized support plans based on their strengths, needs, and priorities.
  • Assist individuals in identifying alternative strategies within their plans when challenges arise.
  • Provide guidance and encouragement to overcome barriers and explore practical alternatives.
  • Offer Step Down appointments, including community outreach, for up to 12 months per person.
  • Arrange one-to-one wellbeing appointments and deliver monthly wellbeing support for up to 12 months, ensuring individuals feel confident in their mental health journey.
  • Support individuals in creating wellbeing plans aligned with the Five Ways to Wellbeing framework.
  • Help foster personal resilience and support the development of recovery plans.
  • Identify early opportunities for employment support and provide appropriate assistance.
  • Contribute to wider organizational initiatives and undertake additional duties as required.
  • Coordinate and attend ward rounds at hospitals to promote services.
  • Link with secondary mental health services to promote referral pathways and services.
  • Encourage individuals to become Experts By Lived Experience to help shape services.
  • Record and evidence outcomes achieved by individuals.
  • Meet monthly targets for wellbeing plans and reviews set by the manager.
  • Use database systems to monitor performance and provide reports to stakeholders.
Person Specification
Experience
  • Relevant experience in health, social care, or voluntary organizations. Knowledge of mental health conditions and recovery interventions such as counselling and community signposting. Understanding of safeguarding policies, GDPR, and confidentiality. Commitment to diversity and equality.
  • Strong listening and verbal communication skills. Training in fields like counselling, mediation, advocacy, or social work is preferred.
