Location: York Street Health Practice, Kirkgate, Leeds LS2 7DJ – Accessible via all public transport links.
Working Hours – Monday to Friday 9am until 5pm (may include late clinics until 7pm or time off in lieu)
About the role…
An exciting opportunity has become available to join the Forward Leeds Active Recovery Team at York Street Health Practice, supporting highly vulnerable groups, including homeless individuals.
The main focus is supporting service users within your caseload, working closely with the Forward Leeds Street Support Team, existing Recovery Coordinators, healthcare workers, and clinical prescribers to build recovery capital for those registered at York Street Health Practice with substance misuse issues, homelessness, or no fixed abode.
The primary aim is to support vulnerable adults in building stability and working towards recovery from substance misuse and related concerns. The role is based at York Street Health Practice but is also integrated with the Active Recovery Team at the Forward Leeds Kirkgate Hub.
