In this role, you will work to facilitate access to treatment for those on the mental health pathway and ensure high-quality continuity of care when transitioning between services

CGL Luton is an innovative and forward-thinking project with a proven track record of effectively engaging people with complex lives. Our ethos is centred around the principles of harm reduction, providing non-judgmental, fact-based information and advice surrounding drug and alcohol use and the associated risks.

Hours Full-time (37.5 hours per week)

Salary £27,449.53 - £31,529.41 dependent on experience (based on full time hours, pro rata if part time)

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

About the Role

  • Support and provide appropriate interventions which enable individuals to have successful engagements with appropriate services, focused particularly on mental health and substance misuse services.
  • Develop a fast-track referral system into substance misuse services from Primary and Secondary Care services, conducting triage and assessments and providing practical support through warm introductions.
  • Develop outcome-focused joint care plans for an individual assessed as ready to engage in recovery-based treatment, including engagement with multi-agency professionals involved in the individuals care.
  • Advocate for the involvement of family members, service users, and the community.
  • Serve as a voice for service users in accessing partnership services.
  • Work closely with service users to facilitate their social (re)integration, enabling them to lead fulfilling and purpose-driven lives. This includes promoting recovery, resilience, peer support, and self-determination.


What we are looking for

  • Possess knowledge and experience in delivering interventions for service users affected by substances.
  • Working knowledge of substance misuse and mental health issues
  • Excel in both verbal and written communication, with proficient IT skills.
  • Demonstrate proactive qualities, working independently with excellent time management and prioritisation skills.
  • Hold a strong understanding of mental health interventions, services, and best practices.
  • Thrive as a supportive team player with exceptional interpersonal skills, capable of collaborating with a diverse range of professionals, agencies, and internal/external stakeholders.
  • Exhibit the ability to navigate change effectively, always prioritising the needs of service users.
  • Recognise the significance of information governance processes and commit to adhering to all necessary safeguards.
  • Pursue continuous learning opportunities to enhance your professional knowledge and skills, while also contributing to the growth and development of your colleagues.
