Qualifications

  • Experience in substance misuse or related health and social care settings preferred.
  • Organised and detail-oriented with strong administrative skills.
  • Confidence in managing client data and following structured processes.
  • Compassionate communicator able to support clients with sensitivity.
  • Proactive in developing partnerships and collaborating across services.

Responsibilities

  • Oversee all aspects of the Tier 4 referral process.
  • Support clients with applications for inpatient detox and rehab placements.
  • Complete financial assessments and required paperwork.
  • Prepare clients for admission with practical and emotional support.
  • Maintain accurate and up-to-date client files for compliance.
  • Collaborate with the admin lead to communicate changes.
  • Build and maintain relationships with treatment services and rehab providers.
  • Identify trends to help improve outcomes and service delivery.

We are looking for a motivated and organised individual to join our integrated healthcare team as a T4 Co-ordinator. This is a key role supporting adults (aged 18+) with drug and alcohol issues as they access inpatient detox and residential rehabilitation services. Please note this post will cover all areas of East Kent.

You'll play a vital part in our clients' recovery journeys by:

  1. Overseeing all aspects of the Tier 4 (T4) referral process.
  2. Supporting clients with applications for inpatient detox and residential rehab placements.
  3. Completing financial assessments and required paperwork.
  4. Preparing clients for admission, offering both practical and emotional support.
  5. Maintaining accurate and up-to-date client files and databases to ensure compliance and smooth transitions.
  6. Collaborating with the admin lead to communicate any changes to inpatient processes and financial arrangements.
  7. Building and maintaining strong relationships with East Kent treatment services and external rehabilitation providers.
  8. Identifying service trends and strengths to help improve outcomes and service delivery.

Qualifications and skills:
